Media

La Carta dei Servizi della Casa per la Vita «La Speranza» è stata realizzata con il fine di conoscere la funzione e l’organizzazione della Comunità, essere informati della qualità e del valore dei servizi offerti, assicurare la trasparenza dell’attività amministrativa, mettere al corrente gli utenti sulle modalità di accesso e di erogazione delle prestazioni e per agevolare una scelta consapevole.
Si tratta di un documento che permette di conoscere meglio la nostra Struttura, i servizi offerti e gli obiettivi che ci proponiamo, per far fronte in modo sempre esaustivo ai bisogni ed alle aspettative degli ospiti e dei loro familiari.

La Carta dei Servizi non è solo un opuscolo informativo, ma uno Strumento che viene offerto perché possa contribuire allo sviluppo del progetto socio sanitario assistenziale mirato a realizzare una sempre maggior “qualità di vita” degli Ospiti.

Questa Carta dei Servizi viene distribuita agli ospiti, ai familiari, ai tutori, alle Associazioni del volontariato, ai medici di base, ai vari servizi dei Dipartimenti di Salute Mentale e ai Comuni di appartenenza degli utenti. La Soc. Coop. Sociale La Speranza sita in Conversano è un soggetto giuridico privato accreditato istituzionalmente dal Servizio Sanitario Regionale così come previsto dalle Determinazioni Dirigenziali della Regione Puglia N. 150 del 26/05/2011 per la Comunità Alloggio H12, con una copertura assistenziale per 12 ore giornaliere, N. 149 del 26/05/2011 per il Gruppo Appartamento, con una copertura assistenziale per 3 ore giornaliere, e l’autorizzazione del Comune di Conversano (Ba) al funzionamento con determinazione N.1062 del 05/06/2012 e determinazione N.1471 del 06/08/2013 per l’avvenuto ampliamento e iscrizione nel Registro Regionale delle Strutture al N. 886 del 03/08/2012 e successivamente all’ampliamento iscrizione nel Registro Regionale delle Strutture al N.845 del 12/09/2013 per la Casa Per La Vita. Confidiamo nella collaborazione per un miglior benessere di tutti gli ospiti mentre ci proponiamo di offrire un servizio all’altezza delle Vostre aspettative.
